Youth Ministry Activity & Liability Waiver
I, the undersigned parent or legal guardian, understand that participation in activities, events, and programs hosted by South Strand Assembly of God (SSAG) involves inherent risks, including but not limited to physical activity, group interaction, off-site travel, and transportation. I voluntarily give permission for the above-named student(s) to participate in all such activities, events, and programs—including being transported to and from youth events and ministry trips by authorized staff, volunteers, or representatives of the SSAG Team—accepting full responsibility and participation at our own risk. This consent, waiver, and release shall remain in full effect for one (1) year from the date of electronic signature below. 1. Release of Liability & Transportation: I hereby release, waive, and discharge SSAG, its officers, agents, employees, assigns, and anyone acting on its behalf from any and all liability, claims, demands, or causes of action for loss, damage, or injury—including personal injury, illness, or property damage—that may be sustained on church premises, during travel/transportation, or during any church-related activity (on-site or off-site). I accept full responsibility for any personal items lost during travel, and I acknowledge that no medical insurance coverage is provided by SSAG. 2. Medical Authorization: In the event of an emergency, I authorize SSAG leadership, staff, or volunteers to secure and obtain necessary medical, dental, or surgical treatment for my student(s) through a licensed physician or facility, and I accept full financial responsibility for all associated medical and transportation costs. 3. Code of Conduct: I understand that participants are expected to follow all safety rules, respect leaders, wear seatbelts during transit, and uphold the standards of the youth ministry. SSAG reserves the right to send any student home at their parent's expense if they exhibit unsafe, disruptive, or uncooperative behavior. 4. Media & Photo Release: I grant permission for my student(s) to be included in photographs, videos, or digital media taken during activities for church promotional, social media, or ministry purposes, unless I notify the church otherwise in writing.
